The Passport to Health program is ending June 30, 2026, and this website will no longer be available after May 22.

Members will receive information soon about Montana's new Primary Care Montana Program,which begins July 1, 2026, including how to choose a provider using the Montana Healthcare Programs Customer Access Portal:

Members will receive a letter with your unique PIN and instructions to get started.

In the meantime, to choose or change your primary care provider (PCP), please contact the Medicaid/HMK Plus Member Help Line at (800) 362-8312 or (800) 253-4091 (TDD), Monday through Friday, 8:00 a.m. to 5:00 p.m.
